Queensland Country Care Navigation

To assist with your rehabilitation or living with a chronic illness, we established Queensland Country Care Navigation.

Care Navigation is a service provided by Registered Nurses for our Members with Better Hospital (Silver+) cover, Top Hospital (Gold) cover (Closed), or Comprehensive Hospital (Gold) cover (Closed).

The service is aimed at supporting Members who require assistance immediately following a period of time in hospital, or for those living with one or more chronic diseases.

If you qualify for the service, a Care Navigation Care Coordinator will work with you to design a tailored health management plan based on your goals and your support needs at no additional cost to you.

We can work in partnership with your existing health professionals to provide you with on-going care.

Heart failure is a condition where your heart isn’t pumping as well as it should be. When your heart is damaged and isn’t pumping properly it can become bigger, weaker, or stiffer. This change in the way your heart is pumping causes extra fluid to build up in your body and sometimes in your lungs.
