Queensland Country Care Navigation

To assist with your rehabilitation or living with a chronic illness, we established Queensland Country Care Navigation.

Care Navigation is a service provided by Registered Nurses for our Members with Better Hospital (Silver+) cover, Top Hospital (Gold) cover (Closed), or Comprehensive Hospital (Gold) cover (Closed).

The service is aimed at supporting Members who require assistance immediately following a period of time in hospital, or for those living with one or more chronic diseases.

If you qualify for the service, a Care Navigation Care Coordinator will work with you to design a tailored health management plan based on your goals and your support needs at no additional cost to you.

We can work in partnership with your existing health professionals to provide you with on-going care.

If you would like to contact the Queensland Country Care Navigation team call 1300 848 034 or simply complete the contact form and they will be in contact.

Families & Mental Illness: Background

Emotions run high when a family member is diagnosed with a mental health issue. Feelings of guilt can surface from the person diagnosed and from their carer; contrasted with feelings of gratitude at the sacrifices and kindnesses shown. New carers are thrown into a role that most are unfamiliar with. Together, the patient and their carer, embark upon a rollercoaster journey of challenges, extreme emotions and, ultimately, of self discovery.

That’s where health professionals come in. There are plenty of effective treatments for anxiety and depression, and the sooner you seek support, the sooner you can recover.
