Queensland Country Care Navigation

To assist with your rehabilitation or living with a chronic illness, we established Queensland Country Care Navigation.

Care Navigation is a service provided by Registered Nurses for our Members with Better Hospital (Silver+) cover, Top Hospital (Gold) cover (Closed), or Comprehensive Hospital (Gold) cover (Closed).

The service is aimed at supporting Members who require assistance immediately following a period of time in hospital, or for those living with one or more chronic diseases.

If you qualify for the service, a Care Navigation Care Coordinator will work with you to design a tailored health management plan based on your goals and your support needs at no additional cost to you.

We can work in partnership with your existing health professionals to provide you with on-going care.

Diabetes - Type 2: Before Diabetes

For some people with a family history of type 2 diabetes, diagnosis in later life doesn’t come as a surprise. Others can go for years without realising that they are living with the condition. Symptoms of type 2 diabetes can vary from person to person, but can include: tiredness, persistent thirst, frequent urination, numbness. In this 'Speaking From Experience' video clip, participants explain what their life was like prior to knowing they had type 2 diabetes.

Taking Medication

Many people put their health and their lives at risk by not taking their medication correctly.
This animation explains the importance of taking medication as prescribed, and offers a range of suggestions for patients on how to remember to take their medication so that it becomes part of a daily routine.

Carbohydrates and GI

Carbohydrate is found in a variety of food and drink and provides the body with fuel (energy). Many foods containing carbohydrate also provide fibre, vitamins and minerals. Carbohydrate breaks down into glucose during digestion and is absorbed into the blood stream. The type and quantity of carbohydrate you eat will affect your blood glucose levels. Diabetes & mental health

Mental illness is enough to cope with on its own, so being told you have diabetes as well can feel overwhelming at first. It’s important to remember, then, that there are things you can do yourself to manage the condition well, and that there are trained people who can help you do this – for example, by recommending eating and lifestyle changes. Diabetes and driving

It is a privilege to drive a motor vehicle and with it comes major personal and legal responsibilities and liabilities. Driving is a complex skill both physically and mentally and for people with diabetes who drive, extra precautions need to be taken to help maximise road safety. The main hazard for those who take insulin or glucose lowering medications is the unexpected occurrence of hypoglycaemia (hypo) but most episodes can be prevented. View this booklet to learn more.

Your diabetes annual cycle of care

The diabetes annual cycle of care is a checklist for reviewing your diabetes management and general health. Your general practitioner (GP) will do this review to help you and your health professionals manage your diabetes, and to reduce your risk of diabetes-related complications.
